FACILITY USE REQUEST
Between church activity, Jack in the Box Playschool, Panther Cub Club and other organizations, our building is used nearly every single day of the year. We are happy to lend the use of our campus for an abundance of purposes. If you are interested in using our facility, fill out the form below and we will be in touch. Please review the attached facility use terms prior to submitting.
Facility Use Terms:
-No Commercial/Personal Profit Use
-User(s) is/are responsible to see that facilities are returned to the condition they were prior to use
-Groups using the facilities are responsible for damage done to church property. If damage is noted, contact the church office at 573-682-3434
-Alcohol is prohibited anywhere on church grounds
-Smoking is not permitted anywhere inside the church building
-This form does not guarantee your reservation. The church office must approve your request
